Administrative Case Management Services Support Service Promoting Family Choice Across the Continuum of Care Support Service Administrative Case Management provides supportive assistance to insure continuity of care across various clinical and/or community services for identified clients. It assists children and families by coordinating their access and linkage to mental health services and community supports. Program Goals Case Managers will work in conjunction with various clinicians and systems to assist clients in promoting and developing natural supports so that they can function successfully in the community and at home. Together with the family they identify needs and establish goals for the client.
The Case Manager will develop a relationship with the child or adolescent and the family to access and link them through and across the continuum of services and supports available throughout the community.
The Case Manager will promote continuity of care across all life domains. Case Manager's Role Coordinate, assess and link consumers to appropriate services, both clinical and supportive, within this agency or throughout the county. Act as advocate to improve the consumer's life situation. Promote consumer/family choice. Facilitate contact with families, school personnel, community supports and other providers Participate in treatment planning with clinical staff and follow-up planning. Facilitate assistance in arranging transportation, acquisition of benefits, and transitioning children into adult service systems. Special Client Benefits Help getting into health insurance plans. Assistance with resolving financial issues. Help accessing clinical mental health services, such as Outpatient, Wraparound, Family-Based, or School Partnership services. Assistance in accessing community and recreational resources. Connecting with an Educational Advocate to attend school meetings with the family. Building Resources and Respect Case Managers will always abide by the child and the family's right to confidentiality. They will always respect the child and the family's right to Family Choice of services or providers. Case Managers will always respect the right and position of the parent to make decisions for the child and the family's right to privacy. Administrative Case Management Always acts in the best interest of the child and family Supportive in nature. Works to build child and family strengths and knowledge of available resources. Eligibility Documentation of a mental health diagnosis and identification of case management requirements are needed to be eligible to receive Administrative Case Management Services.
